Description:

Manages, supervises, coordinates, evaluates, and develops client care staff to ensure quality care delivery and appropriate case management within the agency scope of services and policies; Nurse Practice Act, state and federal regulations.

Responsibilities:

  • Follows all essential functions and responsibilities of the Registered Nurse job description in addition to those listed below:
  • Manages and directs a team of clinical staff ensuring safe, effective, and appropriate home care services.
  • As delegated, is available at all times during regular business hours and as needed outside of business hours to provide support and assure quality care delivery to home care clients.
  • Receives and coordinates referrals by determining services required and Agency’s ability to meet needs.
  • Provides oversight in making client and personnel assignments.
  • Provides coordination of client care.
  • Assures that client needs are continually assessed. Meets with clinical staff to provide guidance and information related to specific issues.
  • Assures the development, implementation, and updates of the individualized plan of care.
  • Provides direction to clinical staff to ensure that client needs are met and services are provided according to the plan. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals.
  • Collaborates with the Agency Administrator/Director in establishing operating budget needs and priorities for the department.
  • Provides or coordinates 24-hour Agency coverage by Clinical Care Manger, designated Registered Nurses, and/or staffing coordinator if applicable. Provides primary on-call or backup, as needed.
  • Evaluates quality of on-call services.
  • Develops working relationships with other health care professionals in the community and families to identify resources available and to ensure access of information to clients

Requirements:

  • Current, valid Indiana Nursing License
  • Pass a thorough background check
  • Pass a drug test
  • Graduate of an accredited school of professional nursing.
  • Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.
  • Complies with accepted professional standards and practice.
  • Must be a licensed driver with automobile insured in accordance with Agency and state requirements.
  • Current CPR certification.
  • Ability to help cover shared on-call responsibilities.

RESPONSIBILITIES:

  • Supervise, train, and provide guidance to the staff in accordance with state, federal regulations, and Benchmark personnel policies.
  • Participate in direct service for each client as needed.
  • Complete daily, weekly, monthly, and yearly documentation as required by Benchmark, state, and federal regulations.
  • Communicate professionally with families, case managers, behavior consultants, health care personnel, schools, employers, etc.
  • Implement training programs as specified in each client’s ISP.
  • Ensure proper medication/treatment administration and assists in meeting the clients healthcare needs.
  • Plan and implements facility based, age appropriate, recreational activities for clients.
  • Implement required training protocol for new and existing staff.

QUALIFICATIONS:

  • Bachelor's degree preferred
  • Two years' experience working with individuals with developmental disabilities
  • A minimum of 1 – year supervisor experience preferred

RESPONSIBILITIES:
  • Provide case management for the residents in the group homes by administering assessments, overseeing resident program plans and coordinating delivery of all services provided by Benchmark Human Services and community services.
  • Develop and monitor the implementation of client’s individual plans. Complete daily, weekly, monthly, and yearly documentation as required by Benchmark Human Services, as well as state and federal regulations.
  • Monitor and maintain client’s and group home files to ensure that all documentation is current.
  • Plan and participate directly in recreational, therapeutic, and training activities of the clients.
  • Seek input and maintain effective communication with IDT and supervisor.
  • Safeguard client’s rights and coordinate with the designated Human Rights Committee when required.
  • Provide training and supervision to staff in accordance with state and federal regulations, and Benchmark Human Services group home policies and procedures.
  • Monitor overall health care needs including plans, medications, treatment protocols, and the general well-being of clients served.
  • Attend all agency and departmental meetings, client appointments as directed, and chair client specific and house meetings.
  • Provide on-call services and respond to client and house needs.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
  • Supervise, train, and provide guidance to the staff in accordance with state and federal regulations, Benchmark Human Services personnel policies, the Supported Living policy, and procedure manual.
  • Participates as part of a collaborative team in the development of each client’s Individual Support Plan (ISP) and Person Centered Plan (PCP).
  • Participate in direct service for each client.
  • Complete daily, weekly, monthly, and yearly documentation as required by Benchmark and state and federal regulations. Maintain in-home client files according to standards and protocol established.
  • Responsible for following client’s budget and ensuring payment of bills. Assist client in maintaining Medicaid eligibility by scheduling and attending all Medicaid appointments and providing proper documentation.
  • Communicate professionally with families, Case Manager, Behavioral Consultant, health care personnel, schools, landlords, employers, etc.
  • Participate in SL Manager’s meetings, team meetings and other departmental meetings, ISP, Quarterlies, IEP meetings, and other meetings as assigned.
  • Implement training programs as specified in each client’s ISP.
  • Comply with behavior support policies and procedures and implements approved behavior support programs.
  • Participate in the hiring and discipline of assigned staff.
  • Ensure proper medication/treatment administration and assist in meeting the client’s health care needs.
  • Schedule and assist clients in all medical appointments and procedures. Ensure completion of proper follow up.
  • Plan and implement community-based, age appropriate, recreational activities for the clients.
  • Implement required training protocol for new and existing staff.
  • Respond appropriately and promptly to on-call situations.
  • Report any suspected abuse, neglect, or exploitation to supervisor or department head.
  • Maintain employment-required documentation (current auto insurance, driver's license), certification (i.e., First Aid, CPR, Medication Administration, Mandt, etc.), and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state and program requirements and standards.
  • Be able to lift up to 50 pounds or more 1% - 33% of the time. Be able to stand and walk 67% - 100% of the time.
QUALIFICATIONS:
  • Bachelor’s degree preferred.
  • Two years experience working with individuals with mental disabilities.
