General Responsibilities: 
  • Provides Home Health Aide services under the direction of the Clinical Care Manager or Registered Nurse
  • Performs services for clients as necessary to maintain their personal comfort and assist with activities of daily living
  • Perform household services
  • Report clinical information on the client's status
  • May perform simple procedures as an extension of therapy or nursing services
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
Qualifications:
  • Successful completion of a formal certification training program and a written skills test and competency
    • Agency evaluation as required by state and federal requirements.
  • Must not have had a continuous period of 24 consecutive months during which none of the certified services were furnished.
  • Is currently listed in good standing on the state Certified Nurse Aide or Home Health Aide registry.
  • Be at least eighteen (18) years of age.

GENERAL RESPONSIBILITIES:

Under the supervision of the Director, the Community Connections Manager will establish and build relationships with community partners and employers in order to procure community-based opportunities for individuals with disabilities currently receiving prevocational training. A key focus will be on discovery activities such as business tours, volunteerism, job shadowing opportunities, job trials, and other work-based learning experiences.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Build and maintain relationships with area businesses that will create opportunities for individuals served to participate in community-based learning and career exploration experiences.
  • Collect, track and retain information, through both research and direct contact, on what businesses in the community need related to hiring and maintaining employees in their organization.
  • Identify and participate in community networking events in order to create awareness of the programs that exist within AWRC and Benchmark and develop leads for business development opportunities.
  • Develop and administer training for Community Connection Coordinators on protocols for taking clients out into the community.
  • Develop process for identifying client vocational interests and seek out community-based activities that will allow the individual served to pursue discovery activities aligned to their interests.
  • Participate in quarterly and/or annual meetings to provide updates to the individual served and their team regarding progress in community-based learning and career exploration activities, seek input from individual and team, and facilitate discussions with individual and team to determine if the individual is interested in pursuing community-integrated employment.
  • Attendance for work per requirements of program, Benchmark, and client specific needs.
  • Reasonable suspicion of a crime against an ICF/MR resident must be reported to Indiana State Department of Health and Law Enforcement (Elder Justice Act).
  • Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.
  • Comply with all standards to assure the health and safety of all individuals

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Must be able to work with minimal supervision ("self starter")
  • Excellent communication skills with all internal and external stakeholders
  • Ability to contact businesses and promote our program

QUALIFICATIONS:

  • Three (3) years of experience in a leadership, supervision, or management role
  • Effective communication skills, both verbal and written
  • Experience working with adults with disabilities preferred
  • College degree in Human Services or other related degree preferred, but minimum of High School diploma or equivalent required
