Benchmark offers Case Management services for individuals with intellectual and developmental disabilities (ID/DD) and their families or guardians in Alabama, Georgia, South Dakota, and Vermont.
Our experience includes providing case management to children, adults in waiver services, individuals seeking employment and intensive case management, to those experiencing an acute behavioral episode.
The role of a case manager is to help individuals and their families or guardians understand the waiver services they are eligible for and what choices they have in selecting service providers.
If you choose Benchmark for your case management needs, we will do the following:
- Learn About You. We will take time to understand you and what matters most to you. This is part of a process called person-centered planning.
- Create a Personalized Plan. We will use a person-centered approach to create a plan called an Individualized Support Agreement.
- Coordinate with Service Providers. We will work with you and your providers to ensure your needs are met.
- Explain Services and Funding. We will help you understand what services are available to you and how those services can be paid for.
- Help You Choose Providers. We will assist you in selecting services with the agencies of your choosing.
- Monitor Services and Satisfaction. We will check whether the services are working well for you and whether you are satisfied with them.
- Advocate for You. We will represent your interests and help ensure your voice is heard.
Alabama
In Alabama, Benchmark provides Intensive Care Coordination services for children and youth (up to age 21) with autism. We connect families with education, behavioral and mental health support, and other resources. We know it can be difficult to navigate systems of support, and our staff are here to advocate for children and families during IEP meetings and appointments.

Georgia
Benchmark has offered Case Management services in Georgia since 2016. We believe that everyone deserves the best options available to manage life’s opportunities. Our case managers help individuals served and their parents/guardians understand the options available to them, so they can make informed decisions.

Case Management
Benchmark provides case management services for individuals, helping them navigate their options for care and funding resources, advocating for and supporting them in pursuing their goals, and helping them to create a customized plan for their life designed to meet their unique needs and desires.
Family Support
Family Support helps individuals and their families by connecting them with services and resources that promote independence, choice, and community inclusion. We believe services should be guided by the individual and their family, not the system. That’s why Benchmark Family Support Coordinators are trained in the Community of Practice/Charting the LifeCourse Framework, a nationally recognized approach that helps individuals create their vision for a good life. Vermont
Benchmark offers Case Management services throughout Vermont. We advocate for improved lives, supporting individuals served and their parents/guardians as they navigate their options. We get to know the people we serve, using a person-centered approach to develop Individualized Support Agreements (ISAs). Review outcome measures.
Benchmark has an Advisory Committee comprised of the Quality Management team, individuals served, and parents and guardians to advise on program management or operations, quality and responsiveness of services, policies, complaints, grievances, appeals, and resolutions. Review quarterly Advisory Committee meeting notes.
