General Responsibilities:  Support and train direct care staff. Ensure quality of services provided and health and safety to individual's served. Essential Functions and Responsibilities: 
  • Collect and disseminate forms and agency information as determined by the Manager.
  • Help develop and manage Supported Living schedule.
  • Responsible for ensuring individuals home is safe, clean, and in good condition.
  • Help collect and manage individual's financial information.
  • Be sure that Active Treatment is being implemented per the client's ISP.
  • Assist in monitoring individual's health care needs.
  • Ensure that monthly safety drills are conducted and documented.
  • Be available 24/7 except day/weekend off.
  • Be available for all client conferences in assigned home.
  • Attend weekly, bi-weekly, and monthly meetings.
  • Other duties as assigned.
Qualifications: 
  • Valid driver's license and auto insurance.
  • At least 2 year's experience working with individuals with ID/DD strongly preferred.
  • High school diploma or equivalent.

Benchmark is seeking an LPN Healthcare Coordinator to provide healthcare coordination and direct intervention, as needed, for designated individuals in the Supportive Living and Adult Day Services. The nurse will also provide training for unlicensed staff to meet the healthcare needs of individuals served.

Part Time!! 25-30 hrs/week - flexible schedule and mileage reimbursement!!

RESPONSIBILITIES:

  • Provide healthcare and wellness coordination for individuals in accordance with all state and federal regulations.
  • Function as liaison and advocate for individuals’ healthcare with all members of individual medical and interdisciplinary teams.
  • Advocate for client’s wellbeing and inclusion.
  • Train unlicensed staff on medication administration, delegated tasks, treatment protocols, and monitoring
  • Implement and train employees on new and revised health related policies and procedures
  • Participate in and attend assigned medical individuals’ appointments (as directed), individual specific, nurse, departmental, and other meetings as required.
  • Manage transitions of care to and from hospitals, nursing homes, rehab facilities or other agencies
  • Oversee pharmacy and medication needs
  • Maintain health-related records, complete documentation and assessments
  • Develop and update individual specific health risk plans to ensure appropriate medical care for assigned individuals
  • Develop long-term relationships with individuals served
  • Provide staff training to meet the healthcare needs of individuals serve.

QUALIFICATIONS:

  • Current Ohio LPN license.
  • Valid Driver’s License.
  • Experience in Community/Long term care
  • Experience working with individuals with intellectual developmental disabilities and/or mental illness.

RESPONSIBILITIES:

  • Must be willing to supervise 2-3 nurses (total) in Cincinatti, Columbus, St. Mary's, and Lima.
  • Must be able and willing to travel 4-5 days a month.
  • Have the ability to express and exchange ideas through verbal and nonverbal communication with all internal and external stakeholders.
  • Have the ability to effectively understand regulation requirements, multi-task, train staff, and provide clinical oversight.
QUALIFICATIONS:
