Essential Functions and Responsibilities:
  • Provide service coordination for clients in Day Services
  • Assist with client-specific staff training.
  • Ensure client schedules and staff ratios align with Plans of Care (POCs) and meet standards.
  • Maintain certifications in medication administration, CPR, and First Aid.
  • Monitor staff and client schedules daily to ensure productivity and compliance.
  • Support the Director in overseeing all sites and client services.
  • Collect and analyze data to track client progress, program goals, and staff productivity.
  • Ensure documentation meets state, federal, and accreditation standards.
  • Attend all scheduled training sessions and staff meetings.
  • Implement and adhere to protocols, including high-risk plans and seizure management.

Essential Functions and Responsibilities:
  • Document behavioral services provided to clients.
  • Monitor, assess, and intervene with clients, and train staff on behavioral supports and strategies.
  • Maintain systems for collecting objective data on client skills and needs.
  • Manage program-specific information and key data points.
  • Conduct functional assessments annually and as needed.
  • Develop and update Behavioral Support Plans (BSPs) to meet client needs and standards.

Essential Functions and Responsibilities:
  • Collect and distribute forms and agency information as directed by the Residential Manager.

Responsibilities:
  • Provide family and caregiver training on the behavior support plan.
  • Participate as part of an interdisciplinary team in the development and implementation of each individual’s care plans.

  • Provide positive behavioral supports according to the behavior support plans and respond to emergency situations as trained.
  • Complete documentation as required by Benchmark Human Services as well as state and federal regulations.
  • Provide regular written and verbal updates on behavior support plan goals to BCBA or BCaBA.
  • Monitor the general well-being of the individuals served by implementing standardized behavior protocols and health care protocols
  • Assist with personal care as required, ensure health, safety and respect for each individual served.
  • Comply with all standards to assure the health and safety of all staff, families, and individuals that we serve.
