Essential Functions and Responsibilities:

• Provide service coordination for clients in Day Services
• Assist with client-specific staff training.

• Ensure client schedules and staff ratios align with Plans of Care (POCs) and meet standards.
• Maintain certifications in medication administration, CPR, and First Aid.
• Monitor staff and client schedules daily to ensure productivity and compliance.
• Support the Director in overseeing all sites and client services.
• Collect and analyze data to track client progress, program goals, and staff productivity.
• Ensure documentation meets state, federal, and accreditation standards.
• Attend all scheduled training sessions and staff meetings.
• Implement and adhere to protocols, including high-risk plans and seizure management.

Essential Functions and Responsibilities:

• Collect and distribute forms and agency information as directed by the Residential Manager.
• Develop and manage staffing schedules according to group home requirements.
• Ensure the individual's home is safe, clean, and well-maintained.
• Manage and collect individual financial information, such as COH.
• Implement Active Treatment per the client's Person-Centered Service Plan (PSP).
• Monitor healthcare needs, medication administration, and management.
• Oversee diet, meal planning, and shopping.
• Conduct and document monthly safety drills.
• Report any suspected abuse, neglect, or exploitation immediately.
• Maintain documentation, certification, and training to comply with standards.
• Be available 24/7, except for scheduled days off.

The Family Support Specialist will assist and enable infants, toddlers and their families to have access to services provided under Montana’s early intervention program, Montana Milestones, and Family Education Services (FES). This person will also fulfill the role of FSS for the purpose of Part C and FES definitions. Responsibilities: 1. Function in intake and/or ongoing coordinator role. 2. Assist the with eligibility determination, including referral, intake and eligibility determination activities. 3. Conduct the family assessment, including interviewing the family. 4. Collect information on the child’s development, including observations of the child. 5. Coordinate evaluations and assessments. 6. Inform parents of their parental rights. 7. Assist parents of eligible children in planning, meeting, developing, implementing and reviewing an Individualized Family Service Plan (IFSP). 8. Collect the family’s insurance or Medicaid information to facilitate coordination of benefits and services. 9. Maintain accurate, complete and timely electronic and paper file records. 10. Conduct referrals and other activities to assist families in identifying available services and providers. 11. Monitor the delivery of services to ensure the services are provided timely and in accordance with the IFSP. 12. Facilitate a transition plan for children exiting Part C. 13. Perform child find and provider recruitment activities. 14. Coordinate and monitor the delivery of available services. 15. Inform families of available advocacy services. 16. Communicate and coordinate services with medical and health providers. 17. • Master’s degree in Applied Behavior Analyst , Psychology, Social Work or Licensure as a counselor and have coursework in human behavior, coursework and experience in developing behavioral treatment plans and at least 2 years of experience providing behavioral supports to people with intellectual disabilities

Responsibilities:

• Provision of services via assessment, plan development and monitoring of progress towards the outcome, training and support to family members in implementing the IFSP.
• Refer for medical or other professional services necessary in the child's care.
• Environmental consultation to ensure that appropriate adaptations and safety issues for the eligible child have been incorporated as set forth in the IFSP.
• Documentation of services provided as reflected in IFSP.
• Complete necessary documentation to facilitate reimbursement through primary funding sources (i.e., Medicaid, First Steps, etc.)
• Comply with First Steps Provider Agreement in regard to continuing education, etc.
• Promotes activities in the natural environment to enhance implementation of IFSP.
• Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.
• Comply with all standards to assure the health and safety of all individuals

Knowledge, Skills and Abilities:

• Ability to supervise others
• Excellent communication skills and attention to detail
• Strong computer and paperwork skills

Qualifications:

• Valid Driver's License.
