Our Group Home programs assist adults with daily activity skills to gain independence in small personal home settings. Our diverse and dynamic programs can’t operate efficiently without the strength and heart of our interdisciplinary team. We are currently seeking a Registered Nurse (RN) to join our team and ensure the highest quality services are provided to our clients.

- Provide healthcare and wellness coordination for individuals in accordance with all state and federal regulations.
- Function as liaison and advocate for individuals’ healthcare with all members of individual medical and interdisciplinary teams.
- Advocate for their wellbeing and inclusion.
- Train staff on medication administration, delegated tasks, treatment protocols, and monitoring. This will include MTTP.
- Implement and train employees on new and revised health related policies and procedures in a timely manner as directed.
- Oversee pharmacy and medication needs. This may include but is not limited to reviews, obtaining medications, and prior authorizations.
- Order medical supplies from established formularies as applicable for your program.
- Maintain health-related records, complete documentation and assessments per company policy, federal, and state regulations, as applicable.
- Support individuals in obtaining health services appropriate for their preventative, acute, and/or chronic illness needs.
- Administer shots as prescribed by the individuals physician.
- Oversee that the health status and physical conditions of individuals are assessed (hands-on), monitored, reported, and responded to in a timely and appropriate manner.
- Respond to emergency medical situations in a timely manner, as appropriate.
- Participate in and attend assigned medical individuals’ appointments (as directed), individual specific, nurse, departmental, and other meetings
- Develop and update individual specific health risk plans to ensure appropriate medical care for assigned individuals.
- Manage transitions of care to and from hospital, nursing home, rehab facility and/or another agency.
- Provide oversight with physician orders and appointments.
- Advise on potential new admissions in regard to healthcare needs and appropriateness for the program.
- Provide nursing on-call per program requirements.
- Mandatory reporter of any suspected abuse, neglect, or exploitation to the supervisor or Director.
- Ability to express and exchange ideas through verbal and non–verbal communication with all internal and external stakeholders.
- Ability to effectively understand regulation requirements, multi-task, train staff, and provide LPN clinical oversight, as applicable.
- Possess the skills to operate independently and/or in a team environment.
- Be in good standing and able to delegate nursing services.
- RN with current Maryland license.
- Experience in community/long term nursing is preferred.
- Experience working with individuals with intellectual, developmental disabilities and/or mental illness.
- Valid Driver’s License.

- Supervise, train and provide guidance to Direct Support Professionals in accordance with state and federal regulations, Benchmark personnel policies, and the Supported Living policy and procedure manual.
- Ensure a safe environment to individuals served.
- Directly responsible to develop and manage staffing schedules.
- Participates as part of an interdisciplinary team in the development of each persons served Individual Support Plan (ISP) and Person-Centered Plan (PCP).
- Participates in direct service for individuals served.
- Completes daily, weekly, monthly and yearly documentation as required by Benchmark and state and federal regulations.
- Maintains in-home client files according to standards and protocol established. This includes ensuring that monthly safety fire drills are conducted and documented.
- Maintain documentation, certification and training as required to comply with federal, state and program requirements and standards.
- Communicates all repairs needs and required maintenance of the client’s home.
- Assists client in maintaining Medicaid eligibility by scheduling and attending all Medicaid appointments and providing proper documentation.
- Communicates professionally with families, Case Manager, Behavioral Consultant, health care personnel, schools, landlords, employers, etc.
- Associates degree in a related field is preferred. High School diploma/GED is required.
- Two years - experience working with individuals with intellectual disabilities.
- A minimum of 1-year supervisory experience.
- Valid driver’s license and auto insurance.
