Responsibilities: 
  • Serve as a member of the professional team for the crisis support homes.
  • Ensure that all services are documented and meet all criteria for Medicaid waiver billing.
  • Provide direct monitoring, assessment, intervention, and staff training.
  • Maintain system for collecting objective data, program specific information, and quality outcome data.
  • Complete functional assessment to develop and implement BSP. Modify as needed.
  • Maintain effective communication with IDT, team clinician, and supervisor.
  • Attend all agency, departmental, and individual specific meetings as directed.
  • Attend individual psychiatric appointments and inpatient psychiatric discharge meetings.
  • Conduct direct support staff training on implementation of individual behavioral support plans.
  • Assist team clinician in case management functions and discharge planning.
  • Must report any suspected abuse, neglect or exploitation to supervisor or department head.
  • Comply with all standards to assure the health and safety of all individuals.
Qualifications:
  • Master’s degree in behavior analysis, psychology, education, social work or related field.
  • Minimum of 35 hours of training and education in behavior analysis and behavior supports.
  • At least two years’ experience with behavior supports evaluation and services for people with intellectual and developmental disabilities.
  • Certification and ongoing training in crisis intervention curriculum.
  • Valid CPR and First Aid Certification
  • Valid Driver’s License and auto insurance.
  • Must be computer literate
  • Must be experienced and competent in profession and maintain any applicable license, training and/or certifications.
  • BCBA preferred, if not BCBA upon hire then must be actively pursuing certification.

Overview: The LPN will serve as a member of the IDD Transitional Support Home team and is responsible for health and safety of individuals and to coordinate and provide their services. The Transitional Home is used as a measure of last resort for individuals in inpatient hospital facilities with no other community placement options. The goal is to stabilize the individual through nursing and behavioral supports while preparing for a permanent placement. Responsibilities:
  • Work with the treatment team to provide direct care to individuals residing in Transitional Home based upon the individual treatment plan.
  • Care includes providing medical treatments, administering medication and giving injections.
  • Ensures that an inventory of medication is safely and securely maintained.
  • Helps individuals with all activities of daily living.
  • Ensures healthy meals and snacks are provided while following menu plans.
  • Measures and records individual’s vital signs, such as height, weight, temperature, blood pressure, pulse and respiration, blood sugars etc.
  • Documents and reports any changes in individual’s progress or conditions.
  •  Uses positive behavioral support strategies as described in behavior support plans.
  • Assist with the implementation of high intensity services such as screening, evaluation and assessment within the limits of their license.
  • Facilitate individual transition to other supports and/or treatment.
  •  Comply with all standards to assure the health and safety of all staff and individuals that we serve.
  •  Must report any suspected abuse, neglect or exploitation to supervisor or department head.
  •  Attend all scheduled training and staff meetings.

Qualifications:

Our GA Community Action Treatment (CAT) program is recruiting for a Therapeutic Mentor. Program is located in Region 1 and 2 (near Kennesaw area). DESCRIPTION: Georgia Community Action Treatment (CAT) is intended to be a safe and effective alternative for children with serious behavioral health conditions that are creating crisis in the family. Upon successful completion, the family should have the skills and support system needed to maintain improvements made during services.  The goals of the CAT program are to:
  1. Strengthen the family and support systems for youth and your adults to assist them to live successfully in the community;
  2. Decrease out – of – home placements, including psychiatric hospitalizations and Department of Juvenile Justice (DJJ) involvement;
  3. Improve family and youth functioning;
  4. Decrease substance use and abuse;
  5. Transition into age - appropriate services; and
  6. Increase health and wellness.
Pay Range: $19.23 hourly  RESPONSIBILITES:
  • Comply with all standards to ensure the health, safety and respect of consumers we serve
  • Act as a member of the CAT team as scheduled and needed
  • Provide crisis follow-up services within 24 hours of crisis disposition
  • Complete face-to-face follow-up with individuals, as assigned, after crisis response
  • Provide one-on-one sessions to youth and their family to address issues related to daily living, social skills, and appropriate communication techniques
  • Complete documentation as required by Benchmark Human Services and state and federal regulations

  • Complete necessary documentation in accordance with applicable policies and procedures, to meet all regulatory requirements and facilitate reimbursement through primary funding sources.
  • Comply with all standards to ensure the health and safety of all staff and consumers.
  • Provide support to clients and staff working in the Georgia Mobile Crisis Support Program.
 QUALIFICATIONS:
  • Must be a licensed LCSW, LPC, LMSW, LAPC or LMFT

  • Current, valid Georgia licensure as a LCSW, LMSW, LPC or similar.

Benchmark is looking for a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our team in our crisis support homes for adults with IDD in Dalton, GA. RESPONSIBILITIES:
  • Provide supports to clients and staff working in the Benchmark In-home supports and crisis home.
  • Coordinate the short-term out-of-home placements with stakeholders.
  • Develop discharge plan detailing the discontinuation from crisis support services.
  • Follow up with individual’s IDT to ensure that they adhere to crisis-service-recommended support/services.
  • Assist in providing information to the mobile crisis team.
  • Facilitate referrals quickly to prevent escalation of crisis.
  • Establish and maintain record keeping functions; integration with CIS system.
  • Complete necessary documentation to facilitate reimbursement through primary funding sources.
  • Develop and implement safety plans as appropriate.
  • Report any suspected abuse, neglect or exploitation to supervisor or department head
  • Must be willing to flex schedule accordingly to provide services, supports, care and treatment to individuals as required, respond immediately when dispatched on crisis responses and be present to provide supervision to staff.
 QUALIFICATIONS:
  • Must be license LCSW or LPC
  • Must be experienced in profession and maintain any applicable license, training, or and certifications.
  • Valid Driver's License
