The Santa Fe office is seeking a qualified and caring person to work as a Program Coordinator in our Supported Living Homes. Successful applicants will attend paid training to obtain required certifications. GENERAL RESPONSIBILITIES: Ensures quality, health and safety and client centered approaches for individuals in community living services. Supervises, supports and trains staff as they assist individuals served. Ensures compliance with federal, state and local regulations while expanding opportunities for persons served to participate in community life as they choose. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
  • Develop and implement the Individual Service Plan and assist in development of the Plan of Care for each person served.
  • Influencing and procuring Plan of Care that addresses all client needs and articulating elements to appropriate staff.
  • Assess, plan, implement, and review person-centered support plans for people served.
  • Coordinate the delivery of all services provided by Benchmark Human Services and the community.
  • Assist people served in expanding their opportunities to participate in community life as they choose.
  • Provide on-call direct support and coverage (beeper) coordination.
  • Participate in the hiring, training, and supervision of Team Leaders and Personal Assistants.
  • Schedule staff in accordance with Plan of Care and providing emergency coverage.
  • Accurate and timely completion of ISPs, Quarterlies, Addendums, client financials, QA walk-throughs, monthly and daily documentation (logs, red, yellow and orange books, fire and tornado drills, client medical information, MARs, etc.)
  • Be aware of and submit reportable incidents within required time frames.
  • Manage and monitor client finances within required time frames (including ICLBs, checkbooks, RLAs, client financial profiles, etc.)
  • Serve as liaison with external case managers, families, other agencies and internal programs.
  • Work collaboratively with director, nurse, HR, accounting, IT, etc.
  • Must maintain good attendance throughout employment.
  • Comply with all standards to assure the health and safety of all individuals.
QUALIFICATIONS:
  • Bachelor's degree in human services and one (1) year experience working with people with developmental disabilities or five (5) years’ experience
  • Valid driver’s license
  • Access to vehicle in good operating order
  • Valid CPR and First Aid

Successful applicants will attend paid training to obtain required certifications. No experience necessary, learn on-the-job. Schedules: Graveyard shift's: 3 days on and get 4 days off. Day Shift: 4 days on and get 3 days off. Responsibilities Include:
  • Protect and honor the rights of people with disabilities

  • Develop a Positive rapport while serving as a role model to create a safe and caring environment
  • Teach skills to individuals served that increase independence such as eating, cooking, and bathing

Successful applicants will attend paid training to obtain required certifications. General Responsibilities: This job carries with it the obligation to respect the dignity and recognize the potential for growth of every individual who receives or provides services and supports, to advocate for the rights of people we support to pursue their own vision, and to be responsible stewards of our human and financial resources in order to create an environment committed to quality. A commitment to this philosophy of services should be demonstrated as job duties are performed Serves in a lead role in providing support to the individuals served in company programs. Responsible for assisting and monitoring aspects that are related to the health, safety and emotional support of the individuals being served.  Essential Functions and Responsibilities:
  • Monitor compliance with the medication and health services policies and procedures. This is to include ‘Special Medical Instructions that any health coordinator has ordered for the consumer.
  • Monitor safety of individuals served with special emphasis to the tracking and prevention of medication errors. This includes assisting with statistics for the presentation to the Safety Committee.
  • Support people being served to identify any questions or concerns they may have regarding their medical care, appointments, medications, etc.
  • ‘Person-directed’ is the approach to use in any interactions with guardians and consumer’s and team members.
  • Interact with individuals in an emotionally supportive and therapeutic manner, and practice effective techniques for reinforcing appropriate individual behavior.
  • Maintain medical files and assist the nurses with distribution of any updated information for the ‘home’ medical file.
  • Maintain the inventory of medical and nursing forms, and order supplies as necessary.
  • Assist with data entry as needed.
  • Assist with medical appointment scheduling
  • Initiate the required medical forms to document appointments.
  • Medication Administration as needed. Site-rounds to houses to verify medication compliance of the AWM staff.
  • Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.
  • Comply with all standards to assure the health and safety of all individuals.
Qualifications:
  • Reliable transportation

  • College Education preferred with a minimum of 2 years experience preferably in the field of medical or services to developmental disabilities
  • Certified Medical Assistant (MA) License
