Successful applicants will attend paid training to obtain required certifications. No experience necessary, learn on-the-job. Schedule 12 hour shifts (part-time or full-time):

  • Protect and honor the rights of people with disabilities
  • Ensuring a supportive and clean home for people served
  • Develop a Positive rapport while serving as a role model to create a safe and encouraging environment
  • Teach life skills to individuals served that increase independence
  • Document data displaying success of client goals and progression
  • Direct and encourage the participation in beneficial community activities
  • Work independent or with co-workers to enhance the growth of individuals served

General Qualifications:

Responsibilities Include:
  • Monitor compliance with the medication and health services policies and procedures
  • Monitor safety of individuals served with special emphasis to the tracking and prevention of medication errors
  • Support people being served to identify any questions or concerns they may have regarding their medical care, appointments, medications, etc.
  • Interact with individuals in an emotionally supportive and therapeutic manner, and practice effective techniques for reinforcing appropriate individual behavior
  • Maintain medical files and assist the nurses with distribution of any updated information for the ‘home’ medical file
  • Maintain the inventory of medical and nursing forms, and order supplies as necessary
  • Assist with medical appointment scheduling
  • Initiate the required medical forms to document appointments
  • Site-rounds to houses to verify medication compliance of the AWM staff.
Qualifications:
  • Reliable transportation
  • Valid auto insurance and driver’s license
  • College Education preferred with a minimum of 2 years experience preferably in the field of medical or services to developmental disabilities
  • Certified Medical Assistant (MA) License

Responsibilities:
  • Provide nursing coordination and wellness coordination for individuals
  • RN to serve as a liaison and advocate when dealing with physicians, psychiatrists & consultants
  • Implement & train Benchmark staff on new and revised health related policies & procedures
  • Respond appropriately & promptly to on-call & emergency medical situations
  • Responsible for clients’ medical files and all documentation is completed accurately
  • Complete paperwork required to document assigned individuals’ health and related care needs
  • Participate in ISP meetings, nurse meetings, departmental meetings, and other meetings as required
  • Perform annual nursing assessments and develop associated health risk plans
  • Obtain and remain current on any certifications required
Qualifications:

Responsibilities Include:
  • Develop and implement Individual Support Plan (ISP) and assist in the development of the Individual Supported Living (ISL) budget for each person served
  • Ensure quality health, safety, and client centered approaches for individuals in community living services
  • Coordinate the delivery of all services rendered by Benchmark Human Services and other providers
  • Provide on-call services and respond to client and house needs
  • Financial management for house budget and for each client as specified in ISP
  • Serve as a liaison with external and internal programs and family members
  • Accurate and timely completion of Person-Centered Plans (PCP’s), ISP’s, client financial, CQAs, monthly and daily documentation
  • Participate in hiring, training, and supervision of Direct Support Professionals
Qualifications:
  • Bachelor’s degree in related field preferred, but not required
  • Five (5) years experience working with developmentally disabled individuals, and one (1) year experience minimum in management
  • Valid driver’s license & current auto insurance
  • Access to a vehicle in good working order
