Transition Support

Benchmark has partnered with Amerigroup in Georgia on a project designed to enhance systems and processes for children in Psychiatric Residential Treatment Facilities (PRTFs) to transition into community placements. The project includes a comprehensive evaluation of the current system to assess existing PRTF transition practices, identifying barriers to transition to community based placement, prioritizing PRTF staff and foster care provider training needs, and examining the continuum of care and related community supports for children and adolescents in Georgia.
