We work with people at home, at work, and in the community, through residential, employment, crisis response, behavioral, recovery, children’s services and day services. We are recruiting for a Program Coordinator for our Supported Living Program in Santa Fe, NM.

Salary: $40,500 plus bonus
Schedule:
- Full-time hours with ability to cover shifts
- Must be available to work weekends & be on call

Responsibilities Include:
- Develop & implement the Individual Service Plan (ISP) and Plan of Care for each person served
- Ensure quality health, safety, and client centered approaches for individuals in community living services
- Coordinate the delivery of all services provided by Benchmark Human Services and the community
- Provide on-call services and respond to client and house needs
- Schedule staff in accordance with Plan of Care
- Serve as a liaison with external and internal programs and family members
- Accurate and timely completion of ISPs, Quarterlies, Addendums, client financials, QAs, monthly, and daily documentation
- Participate in the hiring, training, and supervision of Team Leaders and Direct Support Professionals
General Qualifications:
- COVID-19 Vaccine (when eligible) per NM Executive Order 2022-010
- Valid Driver’s License
- Access to vehicle in good operating order
- Bachelor’s degree in Human Services and 1 year experience working with people with developmental disabilities or 5 years experience
