Description:
Provides time-limited crisis services that support individuals with developmental disabilities in the community and participate in the daily activities of the 24-hour IDD Crisis Home.

Responsibilities:
- Function within the limits of licensure and educational preparation, and effectively with other members of the health care team.
- Utilize the nursing process under the clinical supervision of the LPN House Manager and Registered Nurse to promote and maintain health, prevent disease and other potential disabilities in order for the individual to obtain optimal health.
- Must be willing to flex schedule accordingly to provide services, supports, care and treatment to individuals as required and be present to provide supervision to staff.
- Work with the treatment team to provide direct care to individuals residing in Crisis Home based upon the individual treatment plan.
- Uses positive behavioral support strategies as described in behavior support plans or behavioral guidelines.
- Care includes providing medical treatments, administering medication and giving injections
- Ensures that an inventory of medications are safely and securely maintained.
- Helps individuals with bathing, dressing, personal hygiene, transferring, standing and walking.
- Observes individuals in Crisis Home.
- Documents and reports any changes in individual’s progress, conditions, such as adverse reactions to medication or treatment, changes in behavior, etc. Notifies appropriate treatment team members and discusses any necessary action.
- Assist with the implementation of high intensity services such as screening, evaluation and assessment within the limits of their license.
- Facilitate individual transition to other supports and/or treatment.
- Enhance communication among staff to promote high quality care.
Qualifications:
- Valid State Nursing License
- Valid Driver’s license required
- Must be experienced and competent in profession and maintain any applicable license, training, or and certifications.
