GENERAL RESPONSIBILITIES: Ensure quality, health and safety and client centered approaches for individuals in community living services. Supervise, support, and train staff as they assist individuals served. Ensure compliance with federal, state, and local regulations while expanding opportunities for persons served to participate in community life as they choose. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
  • Develop and implement the Individual Service Plan and assist in the development of the Plan of Care for each person served.
  • Influence and procure Plan of Care that addresses all client needs and articulates elements to appropriate staff.
  • Responsible for assessment(s), planning, implementation, and review of person-centered support plans for people served.
  • Coordinate the delivery of all services rendered by Benchmark and other providers.
  • Assist people served in expanding their opportunities to participate in community life.
  • Provide on-call direct support and emergency coverage.
  • Participate in the hiring, training, and supervision of Team Leaders, Supported Living Managers, and Direct Support Professionals.
  • Schedule staff in accordance with the Plan of Care.
  • Accurate and timely completion of ISPs, client financials, CQA walkthroughs, monthly and daily documentation (logs books, fire and tornado drills, client medical information, MARs, etc.)
  • Be aware of and submit reportable incidents within the required time frames.
  • Serve as liaison with external case managers, families, other agencies and internal
  • Comply with all standards to assure the health and safety of all individuals.

GENERAL RESPONSIBILITIES: Coordinate and manage the operation of Supported Living homes. Supervise staff and provide a meaningful day for the clients. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Supervise, train, and provide guidance to the staff in accordance with state and federal regulations, Benchmark Human Services personnel policies, the Supported Living policy, and procedure manual. 2. Participates as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP) and Person Centered Plan (PCP). 3. Participate in direct service for each client. 4. Complete daily, weekly, monthly, and yearly documentation as required by Benchmark and state and federal regulations. Maintain in-home client files according to standards and protocol established. 5. Facilitate all repairs and required maintenance of the client’s home. 6. Responsible for following client’s budget and ensuring payment of bills. Assist client in maintaining Medicaid eligibility by scheduling and attending all Medicaid appointments and providing proper documentation. 7. Communicate professionally with families, Case Manager, Behavioral Consultant, health care personnel, schools, landlords, employers, etc. 8. Participate in SL Manager’s meetings, team meetings and other departmental meetings, ISP, Quarterlies, IEP meetings, and other meetings as assigned. 9. Implement training programs as specified in each client’s ISP. 10. Comply with behavior support policies and procedures and implements approved behavior support programs. 11. Participate in the hiring and discipline of assigned staff. 12. Ensure proper medication/treatment administration and assist in meeting the client’s health care needs. 13. Schedule and assist clients in all medical appointments and procedures. Ensure completion of proper follow up. 14. Plan and implement community-based, age appropriate, recreational activities for the clients. 15. Implement required training protocol for new and existing staff. 16. Respond appropriately and promptly to on-call situations. 17. Report any suspected abuse, neglect, or exploitation to supervisor or department head. 18. Maintain employment-required documentation (current auto insurance, driver's license), certification (i.e., First Aid, CPR, Medication Administration, Mandt, etc.), and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state and program requirements and standards. 19. Employment Consultants develop and implement individualized plans to assist people with disabilities in finding and maintaining employment positions in the community. This is a full - time salaried position with benefits also there is a quarterly performance incentive. This role will be located in our South Bend office with travel in the South Bend and Elkhart areas. There is a sign on bonus of $5,000 paid in installments. $1,000 at 30 days, 3 months and 6 months and $2,000 at 12 months. JOB RESPONSIBILITIES:
  • Determine necessary information regarding individuals referred to Benchmark Human Services for employment services to develop a strategy to implement the goal of community based employment through in person and remote communication with clients, family members, former employers, and various other entities.
  • Build relationships and research and retain information related to what businesses in the community need related to hiring and maintaining employees through a variety of methods including internet and direct contact.
  • Match employer expectations and needs to the abilities and strengths of underemployed or unemployed individuals with disabilities.
  • Assist client in interview preparation and transportation.
  • Communicate and meet with referring entities such as State of Indiana Vocational Rehabilitation Counselors in regard to planning and client evaluation and job placement efforts through means of telephone, email, and in person contact as well as written reports.
  • Supervise job coaches as they assist with client-specific job shadowing, assessments, job development and/or training.
