Responsibilities Include:

  • Participate in the development and positive implementation of individual’s plans.
  •  Implement Behavior Support Plans, Replacement Behavior Trials and reinforcement plans consistently, continuously and as written across all environments.
  •  Provide positive behavioral supports according to individual BSPs and provide positive reinforcement when individuals engage in socially appropriate behavior.
  • Respond to emergency situations as trained utilizing all aspects of the Mandt Training.
  • Implement Relational Mandt continuously to ensure a positive, engaged and therapeutic environment.
  • Implement Technical Mandt when safety is compromised (ex: physical aggression, dangerous self-injurious behavior and dangerous property destruction) per training and only as needed.
  • Report any restraints or significant behavior to the on-call as soon as the situation deescalates or if assistance is needed.
  • Assess for injuries post behavior or restraint and notify on-call immediately.
  •  Make recommendations for changes to plans based on best practices, hands on experience and individual’s needs.
  •  Prepare and enter daily behavior data as required by the BC. Document the behavior and the environment in observable and measurable terms.
  • Implement Meaningful Day Schedules and provided continuous active treatment through the ISP, BSP, daily schedules, day programs/work and reinforcement schedules.

  • Develop and implement the Individual Service Plan and assist in development of the Plan of Care for each person served.
  • Influence and procure Plan of Care that addresses all client needs and articulates elements to appropriate staff.
  •  Responsible for assessment(s), planning, implementation, and review of person-centered support plans for people served.
  • Coordinate the delivery of all services rendered by Benchmark and other providers.
  • Assist people served in expanding their opportunities to participate in community life.
  • Provide on-call direct support and emergency coverage.
  • Participate in the hiring, training and supervision of Team Leaders, Supported Living Managers, and Direct Support Professionals.
  • Schedule staff in accordance with the Plan of Care.
  •  Accurate and timely completion of ISPs, client financials, CQA walkthroughs, monthly and daily documentation (logs books, fire and tornado drills, client medical information, MARs, etc.)
  • Be aware of and submit reportable incidents within required time frames.
  • Manage and monitor client finances within required time frames (including checkbooks, RLAs, client financial profiles, etc.).
  • Serve as liaison with external case managers, families, other agencies and internal programs.
  • Work collaboratively with director, nurse, HR, accounting, IT, etc.
  • Maintain employment required documentation (Current D.L. auto insurance, certification (ie. First Aid, CPR, Medication Administration, MANDT, etc.), and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state, and program requirements and standards.
  • Perform other duties as assigned.

GENERAL RESPONSIBILITIES:

Provide services to youth with serious emotional disturbances (SED) with the goal of enhancing

the youth’s level of functioning and quality of life.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Provide one-on-one, face-to-face, habilitation services to identified children with SED as outlined in the Plan of Care.
  • Provide habilitation services only in the youth’s home or approved community-based settings identified by the Child and Family Team (CFT).
  • Maintain professional boundaries with youth and family, and adhere to all rules and regulations regarding services as outlined in DMHA Wraparound Training and Provider Reference Module for Wraparound Services dated July 2018.
  • Complete required documentation, with all required elements, as outlined in the Provider Reference Module for Wraparound Services dated July 2018, including Service Notes and the Monthly Summary Reports. Compliance with due dates must be adhered to.
  • Attend and participate in the CFT monthly meetings.
  • Be responsible for the child’s health and welfare during the provision of services and until the child is returned to care of another responsible caregiver.
  • Follow DMHA guidelines for incident reporting.
  • Attend and participate in 10 hours of professional development training per approval year (based on approval date from DMHA.)
  • Enter billable and non-billable time, as well as service notes, in Provide after every day service is provided. Time must be billed in 15 unit of services, 8 minutes or more must be provided in order to bill.
  • Follow guidelines for Service limits: No more than 3 hours per day or 30 hours per month. Hours do not roll over month to month. Hours can’t be provided during scheduled school hours.

General Responsibilities:

Provide support and assistance to people served who have developmental disabilities. Assist people served in expanding their opportunities to participate in community life as they choose. Essential Functions and Responsibilities:

  • Participate proactively as part of an interdisciplinary team in the development and implementation of each served person's individual plan to achieve identified outcomes.

  • Ensure the individual's health and safety while pursuing community opportunities and at home (or for Day Program, in the classroom).

  • Assist the individual served to acquire, retain or improve skills as identified in the person's ISP.

  • Assist the person served with daily living skills, including but not limited to personal care and household tasks (meal preparation, cleaning, laundry, etc.).

  • Maintain a clean and orderly environment for person(s) served.

  • Practice/participate in safety drills as scheduled.

  • Assist the person served in accessing community resources, shopping, and recreational activities.

  • Administer medications and follow physicians' orders concerning medications and treatments. Assist the nurse in meeting the served individual's healthcare needs.

  • Monitor the general well-being of persons served and follow supervisor/nurse instructions, including all healthcare appointments.

  • Communicate to nurse and supervisor health issues and concerns as they arise regarding individuals served.

  • Provide communication to supervisor(s) regarding persons served, home, day program, or work related needs (as applicable to each individual).

  • Comply with and implement all protocols for individuals including, but not limited to: high risk plans, behavior support plans, dining plans, seizure management plans, positioning schedules, etc. (as applicable to each individual).

  • Follow food management policies and assist persons served in preparing nutritious meals according to their dining plan.

  • Complete proper documentation relating to the served persons' healthcare, including MARs, doctor visit forms, healthcare notes, etc.

  • Complete all required programmatic documentation accurately and on time, including but not limited to daily documentation, injury/illness reports, ISP data sheets, notable occurrences, etc. as required by Benchmark, state, and federal regulations.

  • Attend required trainings and meetings, (i.e. staff meetings, house meetings, etc.).

  • Report any suspected abuse, neglect or exploitation immediately to supervisor or department head. Reasonable suspicion of a crime against an ICF/MR resident must be reported to Indiana State Department of Health and Law Enforcement (Elder Justice Act).

  • Communicate all reportable incidents immediately to their supervisor.

  • Provide receipts to supervisor for expenditures of person(s) served, use of petty cash, etc. as applicable.

  • Maintain employment-required documentation (current auto insurance, driver's license), certification (i.e., First Aid, CPR, Medication Administration, Mandt, etc.), and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state and program requirements and standards.

  • Safely transport individuals served as required.

  • Plan and participate directly in age-appropriate recreational, therapeutic and training activities of the persons served.

  • Prepare and enter daily behavior data as required by the BC. Document the behavior and the environment in observable and measurable terms.

  • Implement Meaningful Day Schedules and provided continuous active treatment through the ISP, BSP, daily schedules, day programs/work and reinforcement schedules.

  • Engage individuals in preferred in home activities continuously. This requires staff to be attentive to individuals and the environment at all times. Staff should not sit when clients are not present. Staff should be engaging in activities or supervision at all times. In down time, the house should be cleaned and paperwork should be completed. Once all work is completed, staff should remain alert and know individual’s whereabouts per the bed check schedule.

  • Assist the individuals in planning and participating in community activities on a regular basis.

  • Provide transportation to clients for daily activities utilizing personal vehicle as directed.

  • Implement formal and informal opportunities for skill development for independence and to meet discharge criteria. ESN placement is time-limited and all efforts should be made to prepare individuals for a less restrictive environment.

  • Participate in incident debriefing to determine factors leading to behavior and development of appropriate staff interactions.

  • Maintain levels of supervision per state requirements and as indicated in the BSP’s for safety.

  • Model appropriate behavior and interactions with individuals and co-workers including not speaking of personal business, work related issues, behaviors of individuals served while on shift. Limit personal phone calls and personal conversations in the home.

  • Respond appropriately and professionally to feedback and maintain or improve employee performance.

  • Participate in training related to support of people with behavioral needs, demonstrate learned skills and participate in competency/fluency demonstrations and testing.

  • Implement Technical Mandt when safety is compromised (ex: physical aggression, dangerous self-injurious behavior and dangerous property destruction) per training and only as needed.

  • Report any restraints or significant behavior to the on-call as soon as the situation de-escalates or if assistance is needed.

  • Assess for injuries post behavior or restraint and notify on-call immediately.

  • Make recommendations for changes to plans based on best practices, hands on experience and individual’s needs.

  • Prepare and enter daily behavior data as required by the BC. Document the behavior and the environment in observable and measurable terms.

  • Implement Meaningful Day Schedules and provided continuous active treatment through the ISP, BSP, daily schedules, day programs/work and reinforcement schedules.

  • Engage individuals in preferred in home activities continuously. This requires staff to be attentive to individuals and the environment at all times. Staff should not sit when clients are not present. Staff should be engaging in activities or supervision at all times. In down time, the house should be cleaned and paperwork should be completed. Once all work is completed, staff should remain alert and know individual’s whereabouts per the bed check schedule.

  • Assist the individuals in planning and participating in community activities on a regular basis.

  • Provide transportation to clients for daily activities utilizing personal vehicle as directed.

  • Implement formal and informal opportunities for skill development for independence and to meet discharge criteria. ESN placement is time-limited and all efforts should be made to prepare individuals for a less restrictive environment.

  • Participate in incident debriefing to determine factors leading to behavior and development of appropriate staff interactions.

  • Maintain levels of supervision per state requirements and as indicated in the BSP’s for safety.

  • Model appropriate behavior and interactions with individuals and co-workers including not speaking of personal business, work related issues, behaviors of individuals served while on shift. Limit personal phone calls and personal conversations in the home.

  • Respond appropriately and professionally to feedback and maintain or improve employee performance.

  • Participate in training related to support of people with behavioral needs, demonstrate learned skills and participate in competency/fluency demonstrations and testing.

  • Maintain a clean and orderly environment by completing housekeeping duties, laundry, cooking etc. Complete assigned chore list as required.

  • Monitor the general well-being of the clients served by implementing health care protocols and administering medications.

  • Follow prescribed diets and food management policies and assists clients in preparing nutritious meals according to the menu.

  • Attend all scheduled training and staff meetings. Participate in the meetings/training to identify concerns or changes with individuals served and staff training needs.

  • Provide communication regarding client, home, or work related needs to supervisor/nurse/BC in a timely manner.

  • Assist with personal care as required. Individuals should be clean, presentable and in clothing that is appropriate.

  • Pass medications per physician’s orders and notify the supervisor of any medication issues or medication errors.

  • Assist the individuals in attending all scheduled appointments. Prepare for all appointments by making sure behavioral triggers have been planned for, individuals are on time to appointments, paperwork is completed and after appointment instructions are communicated to the nurse and/or supervisor.

  • Comply with all standards to assure the health and safety of all staff and clients that we serve. Utilize injury reports, monitoring forms and supervisor notification as required.

  • Must report any suspected abuse, neglect or exploitation to supervisor or dept. head.

  • Reasonable suspicion of a crime against an ICF/ID resident must be reported to Indiana State Department of Health and Law Enforcement (Elder Justice Act).
