Description:

  • Provide case management for the individual's in the group home by administering assessments, overseeing resident program plans and coordinating delivery of all services provided by Benchmark Human Services and community services.
  • Develop and monitor the implementation of client’s individual plans. Complete daily, weekly, monthly, and yearly documentation as required by Benchmark Human Services, as well as state and federal regulations.

  • Safeguard client’s rights and coordinate with the designated Human Rights Committee when required.
  • Provide training and supervision to staff in accordance with state and federal regulations, and Benchmark Human Services group home policies and procedures.
  • Monitor overall health care needs including plans, medications, treatment protocols, and the general well-being of clients served.
  • Attend all agency and departmental meetings, client appointments as directed, and chair client specific and house meetings.

GENERAL RESPONSIBILITIES:

  • Provide nursing coordination and wellness coordination for individuals in accordance with all state and federal regulations.
  • Function as liaison and advocate for individuals when dealing with physicians, psychiatrists, and consultants.
  • Provide health care training to Benchmark staff including medication training.
  • Implement and train employees on new and revised health related policies and procedures in a timely manner as directed.
  • Participate in regular pharmacy reviews with the consulting pharmacist and follow up as needed.
  • Complete all paperwork required to document assigned individuals health and related care needs.
  • Respond to emergency medical situations for assigned department(s) in a timely manner.
  • Responsible for assigned individuals medical files to ensure that all documentation is present and accurate.
  • Participate in assigned individuals ISP meetings, nurse meetings, departmental meetings, and other meetings as required.
  • Complete billing documentation for all individuals receiving wellness coordination.
  • Wellness coordination services will be provided under the clinical supervision of an RN. LPN will submit required documentation to RN for review and approval prior to implementation.
  • Perform annual nursing assessments and develop associated health risk plans to ensure appropriate medical care for assigned individuals. Nurse will update when a change in condition occurs.
GENERAL QUALIFICATIONS:
  • LPN with current Indiana license

POSITION SUMMARY: This individual will be working with campers with and without a diagnosis.  The staff member is responsible for the day to day activities of their assigned participant(s) in the camp program; they will adapt activities to the needs of the participants they are assigned and ensure the full participation and inclusion of the camper(s); provide behavior management as needed and follow behavior plans provided by the Field Supervisor; follows directions of field supervisor and camp director(s); works with other staff members in organizing all camp special events; maintains a safe atmosphere; and communicates effectively with parents, staff, volunteers, director(s) and field supervisor.   • Make recommendations for changes to plans based on best practices, hands on experience and individual’s needs.
  • Prepare and enter daily behavior data as required by the BC. Document the behavior and environment in observable and measurable terms.
  • Implement Meaningful Day Schedules and provided continuous active treatment through the ISP, BSP, daily schedules, day programs/work and reinforcement schedules.
  • Engage individuals in preferred in home activities continuously. This requires staff to be attentive to individuals and the environment at all times. Staff should not sit when clients are not present. Staff should be engaging in activities or supervision at all times. In down time, the house should be cleaned and paperwork should be completed. Once all work is completed, staff should remain alert and know individual’s whereabouts per the bed check schedule.
  • Assist the individuals in planning and participating in community activities on a regular basis.
  • Provide transportation to clients for daily activities utilizing personal vehicle as directed.
  • Implement formal and informal opportunities for skill development for independence and to meet discharge criteria. ESN placement is time-limited and all efforts should be made to prepare individuals for a less restrictive environment.
  • Participate in incident debriefing to determine factors leading to behavior and development of appropriate staff interactions.
  • Maintain levels of supervision per state requirements and as indicated in the BSP’s for safety.
  • Model appropriate behavior and interactions with individuals and co-workers including not speaking of personal business, work related issues, behaviors of individuals served while on shift. Limit personal phone calls and personal conversations in the home.
  • Respond appropriately and professionally to feedback and maintain or improve employee performance.
  • Participate in training related to support of people with behavioral needs, demonstrate learned skills and participate in competency/fluency demonstrations and testing.
  • Maintain a clean and orderly environment by completing housekeeping duties, laundry, cooking etc. Complete assigned chore list as required.
  • Monitor the general well-being of the clients served by implementing health care protocols and administering medications.
  • Follow prescribed diets and food management policies and assists clients in preparing nutritious meals according to the menu.
  • Attend all scheduled training and staff meetings. Participate in the meetings/training to identify concerns or changes with individuals served and staff training needs.
  • Provide communication regarding client, home, or work related needs to supervisor/nurse/BC in a timely manner.
  • Assist with personal care as required. Individuals should be clean, presentable and in clothing that is appropriate.
  • Pass medications per physician’s orders and notify the supervisor of any medication issues or medication errors.
  • Assist the individuals in attending all scheduled appointments. Prepare for all appointments by making sure behavioral triggers have been planned for, individuals are on time to appointments, paperwork is completed and after appointment instructions are communicated to the nurse and/or supervisor.
  • Comply with all standards to assure the health and safety of all staff and clients that we serve. Utilize injury reports, monitoring forms and supervisor notification as required.
  • Must report any suspected abuse, neglect or exploitation to supervisor or dept. head
  • Reasonable suspicion of a crime against an ICF/ID resident must be reported to Indiana State Department of Health and Law Enforcement (Elder Justice Act).

Participates as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP) and Person Centered Plan (PCP). 3. Participate in direct service for each client. 4. Complete daily, weekly, monthly, and yearly documentation as required by Benchmark and state and federal regulations. Maintain in-home client files according to standards and protocol established. 5. Facilitate all repairs and required maintenance of the client’s home. 6. Responsible for following client’s budget and ensuring payment of bills. Assist client in maintaining Medicaid eligibility by scheduling and attending all Medicaid appointments and providing proper documentation. 7. Plan and implement community-based, age appropriate, recreational activities for the clients. 15. Implement required training protocol for new and existing staff. 16. Respond appropriately and promptly to on-call situations. 17. Report any suspected abuse, neglect, or exploitation to supervisor or department head. 18. Maintain employment-required documentation (current auto insurance, driver's license), certification (i.e., First Aid, CPR, Medication Administration, Mandt, etc.), and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state and program requirements and standards. 19.
