GENERAL RESPONSIBILITIES:

Provide health and wellness coordination for individuals in the Supported Living department.  Provide staff training to meet the health care needs of individuals served.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Provide nursing coordination and wellness coordination for individuals in accordance with all state and federal regulations.

  • Function as liaison and advocate for individuals when dealing with physicians, psychiatrists, and consultants.

  • Provide health care training to Benchmark staff including medication training.

  • Implement and train employees on new and revised health related policies and procedures in a timely manner as directed.

  • Participate in regular pharmacy reviews with the consulting pharmacist and follow up as needed.

  • Complete all paperwork required to document assigned individuals health and related care needs.

  • Respond to emergency medical situations for assigned department(s) in a timely manner.

  • Responsible for assigned individuals medical files to ensure that all documentation is present and accurate.

  • Participate in assigned individuals ISP meetings, nurse meetings, departmental meetings, and other meetings as required.

  • Complete billing documentation for all individuals receiving wellness coordination.

  • Wellness coordination services will be provided under the clinical supervision of an RN. LPN will submit required documentation to RN for review and approval prior to implementation.

  • Perform annual nursing assessments and develop associated health risk plans to ensure appropriate medical care for assigned individuals. Nurse will update when a change in condition occurs.

  • Obtain and remain current on any certifications requested such as Mantoux Certification, and annual Benchmark nurse skill check-off.

  • Other duties as assigned.

  • Comply with all standards to assure the health and safety of all individuals.

  • Report to work as scheduled.

KNOWLEDGE, SKILLS, AND ABILITIES:

GENERAL RESPONSIBILITIES:

Coordinate and manage the operation of Supported Living homes. Supervise staff and provide a meaningful day for the clients.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Supervise, train and provide guidance to the staff in accordance with state and federal regulations, Benchmark personnel policies, and the Supported Living policy and procedure manual.

  • Participates as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP) and Person Centered Plan (PCP).

  • Participates in direct service for each client.

  • Completes daily, weekly, monthly and yearly documentation as required by Benchmark and state and federal regulations. Maintains in-home client files according to standards and protocol established.

  • Communicates professionally with families, Case Manager, Behavioral Consultant, health care personnel, schools, landlords, employers, etc.

  • Participates team meetings and other departmental meetings, PCPISP, IEP meetings, and other meetings as assigned.

  • Is able to implement training programs as specified in each client’s PCPISP.

  • Complies with behavior support policies and procedures and implements approved behavior support programs.

  • Participates in the hiring and discipline of assigned staff.

  • Ensures proper medication/treatment administration and assists in meeting the client’s health care needs.

  • Schedules and assists clients in all medical appointments and procedures. Ensures proper follow up is completed.

  • Plans and implements community based, age appropriate, recreational activities for the clients.

  • Implements required training protocol for new and existing staff.

  • Responds appropriately and promptly to on-call situations.

  • Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.

Essential Functions and Responsibilities:

  • Assist the individual served to acquire, retain or improve skills as identified in the person's ISP.

Provide services to youth with serious emotional disturbances (SED) with the goal of enhancing

  • Provide habilitation services only in the youth’s home or approved community-based settings identified by the Child and Family Team (CFT).
  • Maintain professional boundaries with youth and family, and adhere to all rules and regulations regarding services as outlined in DMHA Wraparound Training and Provider Reference Module for Wraparound Services dated July 2018.
  • Complete required documentation, with all required elements, as outlined in the Provider Reference Module for Wraparound Services dated July 2018, including Service Notes and the Monthly Summary Reports. Compliance with due dates must be adhered to.
  • Attend and participate in the CFT monthly meetings.
  • Be responsible for the child’s health and welfare during the provision of services and until the child is returned to care of another responsible caregiver.
  • Follow DMHA guidelines for incident reporting.
  • Attend and participate in 10 hours of professional development training per approval year (based on approval date from DMHA.)
  • Enter billable and non-billable time, as well as service notes, in Provide after every day service is provided. Time must be billed in 15 unit of services, 8 minutes or more must be provided in order to bill.
  • Follow guidelines for Service limits: No more than 3 hours per day or 30 hours per month. Hours do not roll over month to month. Hours can’t be provided during scheduled school hours.

GENERAL RESPONSIBILITIES: Provide overall behavioral supports to clients and staff in ESN group homes. Must be willing to flex schedule according to client's needs. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Provide a minimum of ten hours of behavioral supports per site per week. 2. Provide documentation of the behavioral services provided during the designated 10 hours. 3. Provide direct monitoring, assessment, intervention, and staff training including behavioral supports, proactive strategies, triggers, staff interventions, replacement behaviors, etc. 4. Maintain system for collecting objective data regarding the skills and needs of clients served. 5. Maintain system for collecting program specific information including key data points. 6. Position Summary: To provide professional equestrian instructions and leadership to students of all ages and abilities. Responsibilities:
  • Instruct progressive riding classes in English or Western seats, horsemanship, equitation, trail, jumping, vaulting, dressage, therapy or other aspects of horses.
  • Supervise students at all times during their lesson.
  • To provide and maintain a clean and safe riding atmosphere.
  • To attend all meetings, training sessions and facility improvement days.
  • To become certified and maintain a certification through CHA or PATH, Intl.
  • Enforce all policies and procedures adopted by this agency
  • Comply with all standards to assure the health and safety of the clients and staff.
  • Maintain good attendance while employed
  • To organize and teach responsible horse care and management
  • Organize equipment and teach animal care responsibilities.

Qualifications: 
  1. In depth horse knowledge and experience: minimal 6 years riding experience.
  2. CHA Certification or PATH, Intl Certification a plus
  3. Minimal High School diploma or GED
  4. Ability to work outside in a barn atmosphere for up to 8-10 hours a day.
  5. Hardworking and self motivated.
  6. Ability to work without direct supervision.
  7. Must be safety conscious
