Description:
GENERAL RESPONSIBILITIES: Coordinate the orderly delivery of services between the Residential Program, other Benchmark Human Services programs, and community services with which the resident may be involved. Coordinate and manage the operation of group homes and supervise staff. Must be willing to flex schedule according to client’s needs. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
  1. Provide case management for the residents in the group homes by administering assessments, overseeing resident program plans and coordinating delivery of all services provided by Benchmark Human Services and community services.
  2. Develop and monitor the implementation of client’s individual plans. Complete daily, weekly, monthly, and yearly documentation as required by Benchmark Human Services, as well as state and federal regulations.
  3. Monitor and maintain client’s and group home files to ensure that all documentation is current.
  4. Plan and participate directly in recreational, therapeutic, and training activities of the clients.
  5. Seek input and maintain effective communication with IDT and supervisor.
  6. Safeguard client’s rights and coordinate with the designated Human Rights Committee when required.
  7. Provide training and supervision to staff in accordance with state and federal regulations, and Benchmark Human Services group home policies and procedures.
  8. Monitor overall health care needs including plans, medications, treatment protocols, and the general well-being of clients served.
  9. Attend all agency and departmental meetings, client appointments as directed, and chair client specific and house meetings.
  10. Provide on-call services and respond to client and house needs.
  11. Must report any suspected abuse, neglect or exploitation to supervisor or department head.
  12. Reasonable suspicion of a crime against an ICF/MR resident must be reported to Indiana State Department of Health and Law Enforcement (Elder Justice Act).
  13. Other duties as assigned.
  14. Comply with all standards to assure the health and safety of all individuals.
  15. Report to work as scheduled.

Benchmark Human Services is seeking a Program Manager for a salaried position immediately.

GENERAL RESPONSIBILITIES: Coordinate and manage the operation of Supported Living homes. Supervise staff and provide a meaningful day for the clients.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Supervise, train, and provide guidance to the staff in accordance with state and federal regulations, Benchmark Human Services personnel policies, the Supported Living policy, and procedure manual. 2. Participates as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP) and Person Centered Plan (PCP). 3. Participate in direct service for each client. 4. Complete daily, weekly, monthly, and yearly documentation as required by Benchmark and state and federal regulations. Maintain in-home client files according to standards and protocol established. 5. Facilitate all repairs and required maintenance of the client’s home. 6. Responsible for following client’s budget and ensuring payment of bills. Assist client in maintaining Medicaid eligibility by scheduling and attending all Medicaid appointments and providing proper documentation. 7. Communicate professionally with families, Case Manager, Behavioral Consultant, health care personnel, schools, landlords, employers, etc. 8. Participate in SL Manager’s meetings, team meetings and other departmental meetings, ISP, Quarterlies, IEP meetings, and other meetings as assigned. 9. Implement training programs as specified in each client’s ISP. 10. Comply with behavior support policies and procedures and implements approved behavior support programs. 11. Participate in the hiring and discipline of assigned staff. 12. Ensure proper medication/treatment administration and assist in meeting the client’s health care needs. 13. Schedule and assist clients in all medical appointments and procedures. Ensure completion of proper follow up. 14. Plan and implement community-based, age appropriate, recreational activities for the clients. 15. Implement required training protocol for new and existing staff. 16. Respond appropriately and promptly to on-call situations. 17. Report any suspected abuse, neglect, or exploitation to supervisor or department head. 18. Maintain employment-required documentation (current auto insurance, driver's license), certification (i.e., First Aid, CPR, Medication Administration, Mandt, etc.), and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state and program requirements and standards. 19. Performs other duties as assigned. 20. Must maintain good attendance throughout employment. 21. Other duties as assigned. 22. 8. Performs daily, weekly and monthly vehicle/tow motor inspections for preventative maintenance needs and makes recommendations for repair as requested/ required.

9. Maintain and organizes warehouse.

Supported Living Nurse

Benchmark is a national leader in providing programs to individuals with disabilities and mental illness. The Supported Living program is seeking qualified and caring people to work as a Supported Living Nurse. 

GENERAL RESPONSIBILITIES:

  • Provide health and wellness coordination for individuals in the Supported Living department.
  • Provide staff training to meet the health care needs of individuals served.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Provide nursing coordination and wellness coordination for individuals in accordance with all state and federal regulations.
  • Function as liaison and advocate for individuals when dealing with physicians, psychiatrists, and consultants.
  • Provide health care training to Benchmark staff including medication training.
  • Implement and train employees on new and revised health related policies and procedures in a timely manner as directed.
  • Participate in regular pharmacy reviews with the consulting pharmacist and follow up as needed.
  • Complete all paperwork required to document assigned individuals health and related care needs.
  • Respond to emergency medical situations for assigned department(s) in a timely manner.
  • Responsible for assigned individuals medical files to ensure that all documentation is present and accurate.
  • Participate in assigned individuals ISP meetings, nurse meetings, departmental meetings, and other meetings as required.
  • Complete billing documentation for all individuals receiving wellness coordination.
  • Wellness coordination services will be provided under the clinical supervision of an RN. LPN will submit required documentation to RN for review and approval prior to implementation.
  • Perform annual nursing assessments and develop associated health risk plans to ensure appropriate medical care for assigned individuals. Nurse will update when a change in condition occurs.
  • Obtain and remain current on any certifications requested such as Mantoux Certification, and annual Benchmark nurse skill check-off.
  • Other duties as assigned.

Benchmark Human Services is a nationally recognized human services agency assisting people with intellectual disabilities and mental health conditions for over 60 years.  Our Corporate Office is looking for a Front Desk Receptionist.  This position pays $12-$14/hour depending upon experience.
The Receptionist has assigned duties relating to answering the switchboard, relaying messages, and directing visitors to appropriate personnel in addition to administrative duties. Administrative duties include, but are not limited to: creating, editing, and formatting documents, spreadsheets, databases, mailing lists/labels, and presentations, data entry, supply orders, filing, taking minutes (as needed), and cleaning.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
  • Perform reception desk duties on a daily basis, as well as provide coverage during breaks, absences, and vacations of co-worker. Duties include answering multi-line switchboard, directing calls, faxes, messages, and mail to appropriate personnel, and directing visitors to appropriate staff.
  • Communicate information in a professional and courteous manner to individuals served, staff, and visitors. Refer questionable situations to appropriate personnel and communicate any concerns with supervisor.
  • Unlocking and locking the building prior to and at the close of business hours (8:00 a.m. – 4:30 p.m.) on weekdays, even during inclement weather.
  •  Alert personnel and contact 911 in the event of emergencies (actual or drills). Assist staff and clients in exiting the building and take a head count once everyone is outside.
  • Recording staff's absences from the agency and time of return. Informing appropriate supervisors of staff's absences. Maintain visitor log.
  • Maintaining and updating lists of staff and individuals served locations for efficient communication, including phone extension lists and staff/individuals served rosters.
  • Assist with and perform administrative duties as assigned, including but not limited to filing, copying, faxing, shredding, typing, distributing mail, RFP research, creating/editing/maintaining spreadsheets, charts, presentations, forms, mailing labels, databases and documents. Enter Incident Reports into Navision. Assist staff with creating and/or formatting documents in Microsoft and Open Office.
  • Assist HR staff as needed with reference calls, alphabetizing, filing, and projects.
  • Maintain inventory and place office, breakroom, and medical supply orders, including first aid kits.
  •  Maintain conference room, conference call lines, and Zoom schedules. Assist in coordinating meetings – including collecting RSVP’s, creating nametags, assembling packets, placing lunch orders, and setting up equipment.
  • Attend meetings to record, type, format, proof, and distribute minutes as needed. Type and format survey comments for distribution to Senior Staff as needed.
  • Act as Bluffton Road Record Retention Facility contact/back-up.
  • Maintain cleanliness of office. Check for needed cleaning of microwave and refrigerator on a weekly basis, in addition to daily COVID cleaning.

GENERAL RESPONSIBILITIES:

  2. Implement Behavior Support Plans, Replacement Behavior Trials and reinforcement plans consistently, continuously and as written across all environments.

  3. Provide positive behavioral supports according to individual BSPs and provide positive reinforcement when individuals engage in socially appropriate behavior.

  4. Respond to emergency situations as trained utilizing all aspects of the Mandt Training.

  5. Implement Relational Mandt continuously to ensure a positive, engaged and therapeutic environment.

  6. Implement Technical Mandt when safety is compromised (ex: physical aggression, dangerous self-injurious behavior and dangerous property destruction) per training and only as needed.

  7. Report any restraints or significant behavior to the on-call as soon as the situation de-escalates or if assistance is needed.

  8. Assess for injuries post behavior or restraint and notify on-call immediately.

  9. Make recommendations for changes to plans based on best practices, hands on experience and individual’s needs.

  10. Prepare and enter daily behavior data as required by the BC. Document the behavior and environment in observable and measurable terms. 

  11. Implement Meaningful Day Schedules and provided continuous active treatment through the ISP, BSP, daily schedules, day programs/work and reinforcement schedules.

  12. Engage individuals in preferred in home activities continuously. This requires staff to be attentive to individuals and the environment at all times. Staff should not sit when clients are not present. Staff should be engaging in activities or supervision at all times. In down time, the house should be cleaned and paperwork should be completed. Once all work is completed, staff should remain alert and know individual’s whereabouts per the bed check schedule.

  13. Assist the individuals in planning and participating in community activities on a regular basis.

  14. Provide transportation to clients for daily activities utilizing personal vehicle as directed.

  15. Implement formal and informal opportunities for skill development for independence and to meet discharge criteria. ESN placement is time-limited and all efforts should be made to prepare individuals for a less restrictive environment.

  16. Participate in incident debriefing to determine factors leading to behavior and development of appropriate staff interactions.

  17. Maintain levels of supervision per state requirements and as indicated in the BSP’s for safety.

  18. Model appropriate behavior and interactions with individuals and co-workers including not speaking of personal business, work related issues, behaviors of individuals served while on shift. Limit personal phone calls and personal conversations in the home.

  19. Respond appropriately and professionally to feedback and maintain or improve employee performance.

  20. Participate in training related to support of people with behavioral needs, demonstrate learned skills and participate in competency/fluency demonstrations and testing.

  21. Maintain a clean and orderly environment by completing housekeeping duties, laundry, cooking etc. Complete assigned chore list as required.

  22. Monitor the general well-being of the clients served by implementing health care protocols and administering medications.

  23. Follow prescribed diets and food management policies and assists clients in preparing nutritious meals according to the menu.

  24. Attend all scheduled training and staff meetings. Participate in the meetings/training to identify concerns or changes with individuals served and staff training needs.

  25. Provide communication regarding client, home, or work related needs to supervisor/nurse/BC in a timely manner.

  26. Assist with personal care as required. Individuals should be clean, presentable and in clothing that is appropriate.

  27. Pass medications per physician’s orders and notify the supervisor of any medication issues or medication errors.

  28. Assist the individuals in attending all scheduled appointments. Prepare for all appointments by making sure behavioral triggers have been planned for, individuals are on time to appointments, paperwork is completed and after appointment instructions are communicated to the nurse and/or supervisor.

  29. Comply with all standards to assure the health and safety of all staff and clients that we serve. Utilize injury reports, monitoring forms and supervisor notification as required.

  30. Must report any suspected abuse, neglect or exploitation to supervisor or dept. head

  31. Reasonable suspicion of a crime against an ICF/ID resident must be reported to Indiana State Department of Health and Law Enforcement (Elder Justice Act).

  32. Actively and honestly participate in all investigations.

  33. Report to work on time and as scheduled. Previous shift staff may not leave shift until appropriate ratios are maintained.

  34. ESN staff will be utilized as needed by Benchmark and will be required to assist at other homes as assigned or respond to other homes, as situations warrant.

  35. Complete all documentation as required by Benchmark Human Services and state and federal regulations.

GENERAL RESPONSIBILITIES:

  • Provide nursing coordination and wellness coordination for individuals in accordance with all state and federal regulations.

  • Participate in assigned individuals ISP meetings, nurse meetings, departmental meetings, and other meetings as required.
  • Complete billing documentation for all individuals receiving wellness coordination.
  • Wellness coordination services will be provided under the clinical supervision of an RN. LPN will submit required documentation to RN for review and approval prior to implementation.
  • Perform annual nursing assessments and develop associated health risk plans to ensure appropriate medical care for assigned individuals. Nurse will update when a change in condition occurs.
GENERAL QUALIFICATIONS:
  • LPN with current Indiana license
  • Experience in community nursing
  • Experience working with individuals with intellectual and/or developmental disabilities and/or mental illness
